ConnexaCare is AZCOMP’s fully managed Chronic Care Management service. Medicare’s care management programs, such as CCM, RPM, and APCM, are designed to reimburse providers for the kind of ongoing, between-visit care that helps patients stay healthier and out of the ER.
The challenge for most practices is that running these programs requires dedicated clinical staff, new processes, and precise CMS documentation – resources most practices don’t have sitting around.
